Job Description
The PRN Nurse Practitioner is responsible for counseling patients on a preventive checkup using a remote (telehealth) consultation platform. While the patient is remote, the NP's hosting the consultations will work in our Catapult Health office in Dallas, TX (Addison area). Catapult’s licensed Nurse Practitioners review each patient’s risk of diabetes, heart attack, and stroke. Additionally, our Nurse Practitioners screen for depression and make referrals based on those results. Catapult Health’s Nurse Practitioners also provide each patient with an individualized action plan based on their checkup results.
